Documents Required for Gazette Name Change Process

AFFIDAVIT
Affidavit of name change issued by the applicant, containing the new and old name and address.
NEWSPAPER
The newspaper in which the name change was published. Witness form containing signatures of the applicant and two witnesses and the material to be published in the gazette.

CD containing a soft copy of the material containing the old and new name of the applicant, used for printing in the gazette.

Types of Name Change
There are different reasons for changing a name and hence the types of name changes are also different.
Simple Name Change
If a person does not like his name or changes his name due to numerology/astrology, then he gets his new name published in the gazette notification. After which his new name becomes legally valid. Gazette is a legal document. Which is a legal certificate of your name.
Name Change and Gender Change
People who physically change their gender have to go through the name change and gender change process to legally establish their new identity. It includes gender change and publication of name change in gazette.
Name Change of Minor
This is the process in which the name of a child below 18 years of age is changed. In the process of name change of minor, the parents of the child are the applicants. It includes publication of name change of minor in the gazette notification based on the parents of the child

Many women want a name change after marriage because they want to add their husband's surname to their name. In this process, women apply for name change in the gazette notification to legally add their husband's surname to their name after marriage.
Update Document with
Gazette Notification for Name Change
When your name change is published in the gazette notification. After that this gazette name change certificate will be used for name change in Aadhaar card. for which the person has to go to his nearest Aadhaar Seva Kendra and fill the Aadhaar name update form. Submit photocopies of some important documents and a photocopy of the gazette name change certificate which will be the proof of the new name of the person, on the basis of which the name will be changed in the Aadhaar card. After filling the form, pay the fee, after that you will get a slip which contains your UTR number. Through which you can track your Aadhaar card name change process. It may take at least 7 days for the name to be updated in the Aadhaar card.
After changing the name in the Aadhaar card through gazette notification, you will have to change the name in the PAN card as well. For this, you can change name in PAN card through any medium online or offline. Go to the official website of the Income Tax Department (UTIITSL.) and submit the form to update the name on your PAN card. In the form, you will have to submit the required documents like gazette name change certificate or Aadhaar card. Keep in mind that only the Aadhaar card with the new name updated from the gazette name change certificate will have to be submitted. The new name will be updated in your PAN card only from the Aadhaar with your new name. Wait for a few days after submitting the form. After your documents are verified, the e-PAN card with the new name will be sent to you by post within 7 days. And after a month, the physical PAN card will also be sent.
After changing name in Aadhaar card and PAN card with the help of Gazette name change, take an appointment at your nearest passport seva kendra to change name in passport and tell the details related to name change. On the given date, take your Aadhaar card and PAN card updated by Gazette, newspaper clipping and Gazette name change certificate with you. There you will fill a form in which you have to submit your passport with your current name and also submit a Gazette name change certificate, Aadhaar card, PAN and other documents. The process will take a few days. After all your documents are verified, a new passport with a new name will be issued by the passport office and will be sent to you by post at your given address within a month.
The name on the driving licence can be changed only after the new name is updated in the Aadhaar card. To update the name on the driving licence, visit the transport department’s website (parivahan.gov.in) and click on the driving licence name change service. Submit your driving licence details and the Aadhaar card with your new name or for the offline process, go to your nearest RTO office and talk to an officer and fill out the form for name change in driving licence. Submit your updated Aadhaar card, Gazette name change certificate, newspaper clipping along with the form. Based on these, the name in your driving licence will also be changed. It will take a few days to get updated and to get the driving licence with the new name.
To change the name in the birth certificate, you will have to show the gazetted name change certificate. On the basis of this, you can change name in birth certificate, which will take a few days, but on showing the copy of the Gazette Name Change, only your new name is accepted. Let’s say if a person needs a birth certificate anywhere but his name at birth is wrong, then after attaching the copy of the Gazette Name Change with his birth certificate, only his new name will be accepted because the Gazette is a legal certificate. This Gazette Name Change Certificate is accepted in all government and non-government offices.
To Name Change in share certificate, meet the transfer agent of the company and apply for a change of name in the share certificate. To update your name, you will have to submit a copy of the gazette name change along with other documents like identity proof, share certificate etc. Gazetted Name Change Certificate is a legal document on the basis of which your new name will be changed in the share certificate, And you will have to fill the stock power form. After which, after verifying your form, the stock transfer agent will issue a new share certificate in your new name. After which you can use that share certificate in the new name.
